1. The rate of change in the birth rates for Orthodox countries is a guess.
2. Life expectancy determines the value of births/1000 at a stable age distribution.
3. The change in death rate is fixed at (h26-d26)/244 to match the modelout file but add flexibility.
4. If the initial energy efficiency is greater than the maximum and the population is decreasing, efficiency is reduced.
      Food and other energy sources are often not included in reported energy consumption, yet they contribute to GDP.
      Energy efficiency, GDP/Quad, will appear very high if GDP>0 and reported energy use is near zero.
5. The US DOE, EIA online reports world energy use in 1990 of 348.2 Quad and 421.51 Quad in 2003, 
      an increase of 1.48% per year. However, global warming may reduce oil, gas, and coal use unless 
      carbon sequestering becomes effective and efficient. It will be safer to reduce the human population
      and get the GDP/capita up without significant increases in energy consumption.
